Instructions for Completing the Admissions Application
To apply to the Laura Rodriguez Medical Assistant Institute, please complete the online application form.
Requirements
- Digital copies of your unofficial transcripts or records of your high school diploma/equivalent
- American Heart Association (AHA) Basic Life Support (BLS) eCard code
- To find your eCard code, please visit the AHA website
- Both the online and hands-on BLS course options must be completed by the beginning of Module 140
- $100 non-refundable application fee
Instructions
To submit your admissions application, complete the online application form. The application form is divided into five sections: About You, Contact Information, Documentation, Work History (optional) and Application Submission. In order to complete each section, you will need to provide the information outlined below.
Section 1: About You
- First and last name
- Date of birth (MM-DD-YYYY)
- Select cohort schedule (full-time or part-time schedule)
Section 2: Contact Information
- Valid email address
- Phone number
- Current address – number, street and apartment number (if applicable)
Section 3: Documentation
- American Heart Association (AHA) Basic Life Support (BLS) eCard Code
- Both the online and hands-on BLS course options must be completed before starting Module 140
- Government issued photo ID
- Unofficial transcripts or records of high school diploma/equivalent
Section 4: Work History (Optional)
- Name of employer
- Company phone number
- Company address
- Work schedule (e.g., full-time, part-time, etc.)
- Start and end dates
- Job title and description
Section 5: Application Submission
After submitting your completed application, you will be asked to pay a $100 non-refundable application fee via PayPal. Once your payment is successfully processed, you will receive an email outlining the next steps in the admissions process. These next steps include completing the following assessments:
- Predictive Index (PI) Behavioral Assessment
- Wonderlic Basic Skills Test

- To discuss additional options for payment of application fee.
- If you paid an application fee during a recent admissions period and are reapplying to the program.
Admissions Policies:
- Meeting the eligibility criteria does not guarantee admission to the program. Final selection will be based on the student’s application, test scores and interview.
- Late and/or incomplete packets will not be considered.
