Loan Forgiveness Program Notary Service
The steps outlined below will assist you in finalizing your enrollment in the Laura Rodriguez Medical Assistant Institute (LRMAI) Loan Forgiveness Program.
Step One:
Read and accept the Terms and Conditions of the Laura Rodriguez Medical Assistant Institute Loan Forgiveness Program Promissory Note.
Step Two:
Print and sign the Loan Forgiveness Program Promissory Note.
Step Three:
Schedule an appointment to notarize your Promissory Note by emailing notary@fhcsd.org.
